1. You must first register with USA Hockey at www.usahockey.com ("Register Online" link on the right side of the web page), prior to proceeding with DBYHL registration. Once your USA Hockey registration is complete, please proceed to the "REGISTER NOW" button at the bottom of this page.

2. Print, complete, sign the following forms and submit the documents listed below to your coach or team manager/mom at your first scheduled practice:

Consent to Treat

Participant Code of Conduct

Copy of Birth Certificate

3. Registration Fees are as follows:

U-9    $125
U-12    $125
U-14    $135
U-18    $135

4. Payment Options:

Payment in full with credit card (includes a convenience fee in addition to registration fee, as shown in the checkout process)

Payment in person/by mail with check/cash/money order (registration fee only, no convenience fee will be charged; however, the convenience fee will still be shown through the checkout process and in your account until such time your final/full payment is received by DBYHL, at which point the convenience fee will be removed from your account -- you will pay only the registration fee)

If you choose to pay by check/cash/money order at the time of registration then change your method of payment later to credit card/e-check, you will be responsible for the full convenience fee shown as part of the balance of your account.
